Perbedaan Fraksi Ejeksi Ventrikel Kiri pada Pasien Penyakit Jantung Koroner dengan Jumlah Stenosis ?1 dan >1 yang Menjalani Tindakan Percutaneus Coronary Intervention

Abstract

Penyakit jantung koroner (PJK) merupakan penyebab utama morbiditas dan mortalitas kardiovaskular. Derajat keparahan PJK, khususnya jumlah pembuluh koroner dengan stenosis signifikan, berperan penting dalam terjadinya disfungsi ventrikel kiri yang tercermin melalui penurunan fraksi ejeksi (EF). Penelitin ini bertujuan untuk menilai perbedaan rerata fraksi ejeksi ventrikel kiri pada pasien PJK berdasarkan jumlah pembuluh koroner dengan stenosis signifikan. Penelitian ini adalah observasional analitik dengan desain potong lintang ini menggunakan data rekam medis pasien PJK yang menjalani coronary angiography (CAG) dan/atau percutaneous coronary intervention (PCI) di RSU Bahteramas Provinsi Sulawesi Tenggara periode Januari–Desember 2022. Pasien dikelompokkan menjadi stenosis ?1 pembuluh (0–1 vessel) dan stenosis >1 pembuluh (2–3 vessel). Perbedaan rerata EF dianalisis menggunakan uji t tidak berpasangan. Sebanyak 96 pasien dianalisis, terdiri dari 61 pasien dengan stenosis ?1 pembuluh dan 35 pasien dengan stenosis >1 pembuluh. Rerata EF pada kelompok stenosis ?1 pembuluh adalah 46,5±14.0%, sedangkan pada kelompok stenosis >1 pembuluh sebesar 42.0±10.5%. Selisih rerata EF sebesar ?4.51% (95% CI ?9.52 hingga 0.50) dan tidak bermakna secara statistik (p=0.077). Meskipun demikian, terdapat kecenderungan penurunan EF seiring peningkatan jumlah pembuluh stenosis. Pasien PJK dengan stenosis >1 pembuluh cenderung memiliki fraksi ejeksi ventrikel kiri yang lebih rendah dibandingkan pasien dengan stenosis ?1 pembuluh.
